El Buen Samaritano Episcopal Mission (“El Buen”) seeks a Bilingual Community Heath Navigator. As part of the team, you will be multifaceted and a versatile community support generalist who assists clients and participants who visit or call El Buen Samaritano for the entire range of support services El Buen offers. Services include health education, food pantry, application assistance, economic assistance, youth/adult, and a variety of other information and referral services. This position will focus on health education and outreach with the local community, delivering culturally competent health education and outreach. You will engage clients to understand their health and social needs and provide information based upon their unique social determinants of health screening. You will encourage behavior changes by providing information that educates and empowers individuals to look at options and decision-making strategies to positively impact their health and that of their families. You will cultivate strong relationships with clients, facilitate connections, and provide other support to influence health outcomes. Along with a great team, this position will office on campus at El Buen headquarters in Austin, TX. This position must be able to work occasional weekends and evenings.What You Will be Doing:
Community Outreach and Education
- Interpret, translate, and/or provide cultural mediation related to health and social services information.
- Assess client needs for health, social, educational services and resources.
- Provide referrals and facilitate connections to community resources, such as social services, public health insurance programs, legal services or housing supports, to address social determinants of health for patients with complex needs.
- Conduct follow-up on referrals provided to determine outcome and provide additional support as needed.
- Attend in-person and virtual community meetings, events, and health fairs to inform community members about available services, application assistance, better understand community issues and concerns, and build relationships with community members.
Health Education and Promotion
- Develops and delivers health promotion education and activities focusing on a wide array of health and social services topics including reproductive and sexual health, behavioral health, nutrition, physical activity, etc.
- Encourages behavior changes by providing information that educates and empowers individuals to look at options and decision-making strategies to positively impact their health and that of their families.
Evaluation
- Understand program measurements per grant requirements or other funding arrangements to ensure that deliverables are met on a weekly/monthly/quarterly basis.
- Effectively track and collect data electronically to demonstrate program effectiveness and allow for ease in reporting outputs and outcomes.
Additional Requirements
- Maintain patient privacy in compliance with HIPAA federal guidelines and El Buen.
- Acts as a resource for other El Buen staff regarding the public benefits application assistance program.
- Ability to work a flexible schedule, including evenings and weekends.
- Must be able to drive to and from meetings, events and to run errands.
- Must possess a valid TX driver’s license with a good driving record; insurable by El Buen’s auto policy.
- Willing to perform other duties as assigned.
- Participate in emergency response operations as needed to address community disasters.
What You Will Bring:
- High school diploma or equivalent required.
- Bilingual and biliterate English/Spanish required.
- Certification as a Community Health Worker or Navigator preferred, and within a year of being hired would be supported in certification as a CHW.
- A minimum of (3) years of experience in health and social services, including reproductive, behavioral, adolescent health, access to food and nutrition, or chronic disease management.
- At least two (2) years of experience of facilitating referrals to community resources for families with limited financial and healthcare resources in Travis County and surrounding areas.
- Client Centered Counseling experience or motivational interviewing experience preferred.
- Work will often require working outside during extreme temperatures (summer and winter) during our food pantry operations.
- Frequent activity requiring the ability to stand, walk and/or reach with hands and arms.
- Proficient in Microsoft Office suite including Word, Excel, Outlook, and PowerPoint.
- Utilize data collection system to track client information, assessments, encounters, and outcomes.
- Effectively communicate and understand how to work with and listen to clients who are under a great deal of stress and anxiety and stakeholders from different socio-economic and cultural backgrounds.
- Adapt to quickly changing business environment including learning and applying new training and knowledge.
- Demonstrate the ability to problem solve, analyze data, make good decisions, attend to details, and manage projects.
- Strong familiarity with the community we serve and strive to understand the socio-economic and cultural environments in which our clients live.
