Our convenient locations provide beautiful state of the art facilities that reflect the rich and scenic splendor the valley.Apply today by sending your resume to Stacee Reed, Provider Recruiter at [email protected] Responsibilities:Conducts Annual Health Assessments including an evaluation of past medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical examPerform evidence-based practice screenings including point of care testing (as appropriate)Identify diagnoses to be used in care management and active medical management in the furtherance of treatmentFormulate a list of current and past medical conditions using clinical knowledge and judgment and the findings of the assessmentCommunicate findings in the assessment that will be used to inform patients primary care provider of potential gaps in careIdentify urgent and emergent situations and intervene appropriatelyEducate patients on topics such as disease process, medication, and compliance Qualifications:Certified: California Family Nurse Practitioner LicenseDEA CertificateBLS or ACLS certificationGraduate of an accredited Family Nurse Practitioner ProgramNational certification in his/her primary care specialtyQualified to practice by certification in the State of CaliforniaExperienced FNP (particularly those who have worked in Health Centers) and those FNP who have completed family practice training are preferredThe base pay range for this exempt position starts at $137,000.00 a year. Our competitive salaries are dependent on knowledge, skills and experience. The base pay does not include other forms of compensation and/or incentives.In addition, our comprehensive benefits package includes:
